Hip Avascular Necrosis (AVN)

Avascular Necrosis (AVN), also known as Osteonecrosis of the Hip, occurs when the blood supply to the ball of the hip joint (femoral head) is reduced or interrupted. Without adequate blood flow, the bone gradually weakens and may collapse, leading to arthritis and persistent hip pain. Early diagnosis offers the best opportunity to preserve the natural hip joint.

What Causes Hip AVN?

AVN may develop following an injury or without any obvious trauma. Common causes and risk factors include:

  • Hip fractures or dislocations
  • Long-term steroid use
  • Excessive alcohol consumption
  • Diabetes
  • Sickle cell disease
  • Autoimmune disorders
  • Certain blood disorders and medical treatments

In many patients, no definite cause is identified (Idiopathic AVN).

Common Symptoms of Hip AVN

Patients may experience:

  • Deep groin or hip pain
  • Pain while walking or climbing stairs
  • Limping or reduced walking distance
  • Stiffness and reduced hip movement
  • Night pain in advanced stages

Symptoms usually worsen as the disease progresses.

Diagnosis of Hip AVN

Early diagnosis is crucial. Evaluation includes:

  • Thorough medical history for risk factors
  • Clinical examination
  • X-rays
  • MRI scan (most sensitive for early-stage AVN)
  • CT scan in selected cases

MRI can detect AVN before changes become visible on X-rays, allowing earlier treatment.

Treatment Options for Hip AVN

Treatment depends on the stage of the disease.

Early AVN (Before Bone Collapse)

The aim is to preserve the natural hip and delay or prevent arthritis. Treatment options include:

  • Activity modification
  • Pain-relieving medication
  • Physiotherapy
  • Protected weight bearing
  • Core Decompression Surgery
  • Core Decompression with PRP or Bone Marrow Concentrate (BMAC) in selected patients

These procedures reduce pressure within the bone, improve blood supply and encourage healing.

Advanced AVN (After Bone Collapse)

When the femoral head has collapsed and arthritis has developed, Total Hip Replacement offers the most reliable solution for pain relief and restoration of mobility.

Modern hip replacement enables most patients to return to an active, independent lifestyle.

Why Early Treatment Matters in Hip AVN

Timely diagnosis and treatment for Hip AVN can:

  • Preserve the natural hip joint
  • Delay or prevent joint collapse
  • Reduce pain and improve mobility
  • Delay the need for hip replacement
  • Maintain an active lifestyle for longer
