What is an Elbow Contracture?

A stiff elbow, also known as an Elbow Contracture, can make even simple daily activities such as eating, dressing, combing your hair or reaching overhead extremely difficult. Loss of elbow movement often develops after an injury, fracture, surgery or arthritis and, if left untreated, may significantly affect your quality of life.

An elbow contracture is a condition in which the elbow loses its normal ability to bend or straighten fully because of tightening of the joint capsule, surrounding muscles, ligaments or scar tissue.

A healthy elbow usually moves through approximately 30° to 145°, allowing comfortable performance of everyday tasks. Significant loss of this range can interfere with activities such as eating, personal hygiene, dressing and lifting objects.

At the Elbow Centre, Dr. Anand Jadhav specialises in the diagnosis and treatment of elbow stiffness, offering both advanced non-surgical care and minimally invasive arthroscopic surgery to restore movement and improve function.

What Causes Elbow Stiffness?

Elbow contractures may develop following conditions that cause inflammation, scarring or damage to the elbow joint. Common causes include:

  • Elbow fractures and dislocations
  • Previous elbow surgery
  • Osteoarthritis
  • Rheumatoid arthritis
  • Loose bodies within the joint
  • Bone spurs (osteophytes)
  • Malunited fractures
  • Joint infections
  • Burns and severe soft tissue injuries

In many patients, stiffness develops gradually as scar tissue forms around the joint during healing.

Symptoms of Elbow Stiffness / Contracture

Patients commonly experience:

  • Difficulty straightening the elbow
  • Difficulty bending the elbow
  • Reduced range of movement
  • Stiffness after injury or surgery
  • Difficulty eating, dressing or personal care
  • Problems reaching overhead or behind the head
  • Reduced arm function

Interestingly, many patients experience stiffness with little or no pain, making loss of movement the primary complaint.

When Should You Seek Specialist Care?

You should consult an orthopaedic specialist if:

  • Elbow movement continues to reduce after an injury or surgery
  • Physiotherapy is no longer improving movement
  • Daily activities become difficult
  • Elbow stiffness persists for several weeks
  • You are unable to fully bend or straighten the elbow

Early treatment often leads to better outcomes and may reduce the need for surgery.

Diagnosis of Stiff Elbow

A comprehensive assessment includes a detailed medical history, clinical examination, measurement of elbow movement, assessment of joint stability and evaluation of muscle strength.

Additional investigations may include:

  • Digital X-rays
  • CT Scan
  • MRI Scan
  • Nerve Conduction Studies (when required)

These investigations help identify the underlying cause of stiffness and guide the most appropriate treatment plan.

Non-Surgical Treatment for Elbow Stiffness

Patients with recent elbow stiffness often improve with conservative treatment.

Physiotherapy

A specialised rehabilitation programme focuses on:

  • Gentle stretching exercises
  • Progressive range-of-motion exercises
  • Joint mobilisation
  • Muscle strengthening
  • Functional training

Splinting

Dynamic or static progressive splints may be used to gradually improve elbow movement over time.

Serial Casting

In selected cases, controlled casting helps stretch tight tissues and improve flexibility.

Manipulation Under Anaesthesia

Occasionally, the elbow may be gently manipulated under anaesthesia to release adhesions and improve movement. This is considered only in carefully selected patients.

When is Surgery Recommended for Elbow Stiffness?

If significant stiffness persists despite dedicated physiotherapy and non-surgical treatment, surgery may be advised. The choice of procedure depends on the cause and severity of the contracture.

Arthroscopic Elbow Capsular Release

Arthroscopic Capsular Release is a minimally invasive (keyhole) procedure performed through small incisions using a high-definition camera and specialised instruments. The procedure aims to:

  • Release the tight joint capsule
  • Remove scar tissue
  • Remove loose bodies
  • Excise bone spurs when necessary
  • Restore elbow movement

Compared with traditional open surgery, arthroscopy offers:

  • Smaller incisions
  • Less soft tissue damage
  • Reduced postoperative pain
  • Faster rehabilitation
  • Earlier return to daily activities

Open surgery may still be recommended in complex cases where extensive reconstruction is required.

Rehabilitation After Surgery

Successful recovery depends on an intensive rehabilitation programme. Following surgery:

  • Pain is controlled with appropriate medication and regional anaesthesia when required
  • Early elbow movement begins as soon as possible
  • Continuous Passive Motion (CPM) may be used in selected patients
  • Physiotherapy focuses on restoring flexibility, strength and function
  • Ice therapy and elevation help reduce swelling
  • Patients are encouraged to maintain a healthy lifestyle and avoid smoking to support healing

Close supervision by an experienced physiotherapy team is essential to maximise the final range of movement.

Are There Any Risks?

Elbow contracture release is generally a safe and effective procedure when performed by experienced surgeons. As with any operation, potential risks include infection, bleeding, swelling, bruising, injury to nearby nerves or blood vessels, residual stiffness, recurrence of contracture and the need for prolonged rehabilitation.

These complications are uncommon, and careful surgical technique combined with dedicated physiotherapy helps minimise these risks.

Frequently Asked Questions (FAQs)

Can elbow stiffness improve without surgery?

Yes. Many patients, especially those with recent stiffness, improve with physiotherapy, splinting and supervised rehabilitation. Surgery is usually reserved for patients who fail to improve with conservative treatment.

How soon should treatment begin?

Early treatment generally produces better results. Delaying treatment can allow scar tissue to mature, making recovery more difficult.

How long does recovery take after arthroscopic release?

Recovery varies depending on the severity of stiffness and patient commitment to physiotherapy. Most patients continue supervised rehabilitation for several weeks to months to achieve the best possible range of movement.
