Chondromalacia Patellae

Chondromalacia Patellae, also known as Patello-Femoral Pain Syndrome, occurs when the cartilage on the underside of the kneecap (patella) becomes softened or damaged, leading to pain and irritation at the front of the knee.

It is commonly seen in young adults, athletes and active individuals, particularly those involved in running, jumping and stair climbing activities.

Common Symptoms of Chondromalacia Patellae

Patients may experience:

  • Pain at the front of the knee
  • Pain while climbing or descending stairs
  • Discomfort after prolonged sitting (“Movie Theatre Sign”)
  • Pain during squatting or kneeling
  • Clicking, grinding or crackling sensations
  • Mild swelling around the knee

What Causes Chondromalacia?

Several factors can contribute to abnormal kneecap tracking and cartilage wear, including:

  • Muscle imbalance or weakness
  • Poor patellar alignment
  • Flat feet
  • Repetitive sports activities
  • Previous knee injuries
  • Patellar instability or hypermobility

How is Chondromalacia Diagnosed?

Diagnosis is usually based on:

  • Patient history
  • Clinical examination
  • X-rays
  • MRI scan in selected cases

These investigations help assess cartilage damage and identify any underlying alignment problems.

What are the Treatment Options for Chondromalacia?

Most patients improve without surgery through:

  • Physiotherapy and muscle strengthening
  • Patellar taping or bracing
  • Activity modification
  • Weight management
  • Foot orthotics for flat feet
  • Anti-inflammatory medications when required

Physiotherapy aimed at improving quadriceps, hip and core strength remains the cornerstone of treatment.

When is Surgery Needed in Chondromalacia?

Surgery is rarely required but may be considered for patients with:

  • Persistent pain despite rehabilitation
  • Significant cartilage damage
  • Patellar maltracking or instability
  • Associated structural abnormalities

Arthroscopic procedures or realignment surgery may be recommended in selected cases.
