Wrist Arthroscopy
Arthroscopy is a minimally invasive technique of visualizing the inside of the wrist joint using a small telescope and attached video camera.
After knee and shoulder joints, the wrist has become the third most common joint to undergo arthroscopy. Wrist arthroscopy allows the surgeon to diagnose and treat many problems of the wrist through a series of very small incisions (portals). Because the incisions used with wrist arthroscopy are smaller and disrupt less soft tissue than conventional open surgery, pain, swelling and stiffness are minimal, the scars are tiny, and the recovery is often faster.

Wrist arthroscopy allows the visualization of the cartilage surfaces of all bones in the wrist and better evaluation of the ligaments between the various bones of the wrist.
It is performed for the following reasons:
- TFCC injuries management
- Interosseous ligament injuries
- Anatomic reduction assistance of distal radius or scaphoid fractures
- Ulnocarpal impaction
- Debridement of chondral lesions
- Removal of loose bodies
- Synovectomy for rheumatoid arthritis
- Excision of dorsal wrist ganglia
- Assistance in treatment of wrist instability and arthritis
- Septic wrist irrigation and debridement
- Diagnosis of unexplained mechanical wrist pain
Wrist arthroscopy is performed under general anaesthesia or a regional block as a day care or overnight stay surgery.
- Patient is in a lying down position with the elbow kept bent at 90 degrees on a special table.
- A special finger traction device is used for joint distraction.This opens up the tight joint spaces and allows arthroscopy instruments to be moved around easily.
- Sterile normal saline fluid is infused into the joint to expand the joint and allow improved visualization during the procedure.
- Tiny skin incisions (portals) are made through pre-marked safe areas.
- A small telescope with attached small video camera is inserted into the wrist joint. The camera lens magnifies and projects the small structures in the wrist onto a television monitor, allowing for more accurate diagnosis.
- Several small incisions are used to allow the surgeon to place the camera in different positions to see different structures inside the joint as well as to place various small instruments into the wrist joint to help diagnose and treat various problems in the wrist.
- Sometimes wrist arthroscopy may need to be combined with open procedures.
- After wrist arthroscopy, the portals are closed with simple sutures or sterile sticky tapes. Then dressing pads are applied and a compression bandage is given. A wrist splint may be given along with a shoulder sling.
- The period of protection will vary depending on what was performed at the time of surgery. The arm is rested over two pillows and kept elevated to reduce swelling and pain. Regular icepacks are given 4 to 5 times a day.
- Wrist and finger exercises are performed as instructed by the physiotherapist.
As with any surgery, wrist arthroscopy has possibilities of some risks and complications. These include – infection, swelling, stiffness and potential damage to nerves, blood vessels and tendons, damage to joint cartilage and ligaments.
Wrist arthroscopy is not appropriate for all wrist conditions and is dependent on the surgeon’s training, expertise and comfort level.
