We request our new patients to come 15 – 20 minutes before their scheduled appointments.
This will enable them to complete their registration formalities as a new patient and fill up their health-related questionnaires.
(Registration process is a necessary step for all patients as it helps us in maintaining your health-related records and retrieval of your previous data is easy.
All your personal and health related information is kept confidential and not shared with anyone. We only use your details for directly communicating with you, whenever needed.
HEALTH RELATED QUESTIONNAIRE:
All NEW PATIENTS (i.e patients coming to see Dr. Jadhav for the first time) are required to fill up their health-related questionnaire before seeing Dr Jadhav.
This helps us to judge your health status and also know about any medications that you may be taking. This information also helps us to provide you safe and effective treatment for your problems.
For FOLLOW-UP VISITS:
Patients need to mention their registration number or their full name at the reception counter. This helps to bring up their detailed records.
Most clinics and hospitals are now providing printed cards to patients with their registration details. In such cases, showing the patient registration card to the receptionist also helps to retrieve your records.
PAYMENT FOR CONSULTATION AT PRIVATE CLINICS:
The fees for consultation with Dr Jadhav and any charges for procedures performed in the clinic, can be paid by cash, debit cards, credit cards or instant online money transfers via several portals. We do not accept cheques for clinic consultation.
Patients are given relevant payment receipts for the services provided at the clinics.

CONSULTATION PROCESS:
Patients can bring along any family member or a close friend when coming for consultation with Dr Jadhav. This can help ease patient’s stress or anxieties about clinic visits. Accompanying persons can also help patients in recollecting any health-related issues which they may have forgotten. Patients can decide if their accompanying person can be present in the examination chamber during their consultation and discussions about their treatment. If patients want to be examined and counselled alone, then their accompanying person is requested to sit in the waiting area. This is to ensure privacy of the patient during their examination and any treatment.
